ETF Reporting | Service Information Form

Please use the following form to complete a service inventory of the services provided by your organization with the use of State of Alabama, Education Trust Fund dollars. A new form should be completed for each service provided during the required reporting period.

Please note that a service is defined as a distinct practice, treatment, program, tool, etc. that attempts to target one or more distinct populations. Examples of distinct services could be an educational course designed for middle school girls (program), operating a museum (service), or educational mobile app (tool).

House Bill 187 (Act 2020-169) was passed by the Legislature and signed by the Governor during the 2020 Regular Session.

The act identifies several entities in Section 3 that are required to “file a report with the Chairs of the House Ways and Means-Education Committee and the Senate Committee on Finance and Taxation-Education and the Deputy Director of the Legislative Services Agency-Fiscal Division on the first day of the 2020 Regular Session regarding the expenditures and usage of the funds appropriated in fiscal year 2021 as provided in Section 15.” 

Section 15 of the act requires  nongovernmental entities to report to the same people on the “proposed expenditures and usage of funds appropriated in the fiscal year ending September 30, 2021.”

Those reports are also required to include a detailed service inventory. The same information is also required of any entities that were also allocated funds pursuant to Act 2019-403.
